[Abawdnavigators] Referral form samples

Hello Navigators,

I want to thank the group of volunteers who worked on the referral form.  What they concluded is that the primary use of the referral form is for the ABAWD.  It can also be sent to the referred program if the form would add additional information beyond the IEP that the provider might need.  We came to consensus on the common information that should be there, and then made some suggestions for what else can be on the form.  Attached to this email is the Referral Form Summary and two examples of possible forms.  It is important that you figure out what works best for you, the ABAWD and your community.  You are welcome to use either of these forms and adapt to your program or create your own using the information provided.
