[LIBRARYDIR] Book donations procedures and forms

Hi all,

Skagit is looking at procedures around accepting donations of books and other materials for the library.  We want to re-write our donations receipt.  I would really appreciate seeing what form(s) others have created that you are using to acknowledge receipt of a gift/donation.  Do you keep a copy for your files?  What do you do with copies that you keep?  How do you acknowledge or thank your donors?
