[Wsssc] Check in about student attestation forms

We are only using the exemption forms for student employees and regular employees.  We only verify vaccination status in employees and student employees.  Students just have to declare which vaccine they received and the date(s) on which they were administered so we can determine if they are fully vaccinated.  Students (not student employees) declaring an exemption just have to check a box.

Hi Folxs,

I wanted to check in if anyone around the state has developed or has received (?) attestation forms for students. Currently we have only received the employee ones, see attached. Could you all let me and I am sure others will want to know, if you have developed one? Also are you requiring students to complete the exemption forms? What I understand is that the state is not requiring exemption forms from students, but they do need to identify which exemption they have. Finally are folxs asking students to provide proof of vaccine beyond the attestation form? Again my understanding is that we are being told by the state that we can't collect or store them... any insight or feedback you all have? Thank you in advance!
