We just re-did ours at Evergreen.

We accept small donations on the fly and have forms printed out at the desk for people to fill out. Every donation large and small requires a form. We save the forms in paper, for now, but may at some point scan them and ditch the paper.

Feel free to steal whatever looks useful.

Hi all,

Skagit is looking at procedures around accepting donations of books and other materials for the library.  We want to re-write our donations receipt.  I would really appreciate seeing what form(s) others have created that you are using to acknowledge receipt of a gift/donation.  Do you keep a copy for your files?  What do you do with copies that you keep?  How do you acknowledge or thank your donors?
