[LIBRARYDIR] Chromebook checkout forms?

Ahniwa,

We use Dell laptops rather than Chromebooks but the principle is the same.  I've attached the form we use.

Hey folks!

We are getting our first batch of chromebooks to check out to students this quarter. We're getting 20 of them and plan to check them out for the entire quarter. If it goes well, we will likely purchase more.

We're working on forms that students will need to sign, outlining responsibilities and use. Do any of you have something similar you could send my way so that we might benefit from your good ideas?
