GHC's policy is pasted below.  We had a rudimentary procedure attached to it, but are soon to be adopting UoM's procedures, edited to fit our environment.

Purpose
Grays Harbor College is committed to providing equality of opportunity to persons with disabilities including equal access to college information, programs, services, and activities provided through information technology (IT).  The college seeks to deploy information technology that has been designed, developed, or procured to be accessible to people with disabilities, including those who use assistive technologies. Policy
Grays Harbor College is committed to providing equitable access to information, programs, and activities through its information technologies, web pages, web-based applications, operating system-based applications, online instructional content, services, and resources for all its constituencies.  This policy applies to the development, procurement, and implementation of technologies and content, including websites, hardware, and software, currently in use, or being evaluated for purchase or adoption.

Hopefully most of you have gotten your policies developed and approved. Anyone mind sharing their's with the group? A few folks have mentioned that they'd like to see what other schools have come up with policy-wise.
