[Abawdnavigators] Information you need to have on ABAWDS

Hi All,

The contract is waiting for one more signature and we should be all set.  Now that we have the contract 99% done I wanted to make sure you all have the following list of records/forms you are required to keep on file.  Below is the list:


Record requirements:

(1) Colleges will maintain client information records to include at minimum:

(a) First and last name of the client

(b) DSHS Client Identification Number

(c) Date of Birth

(d) Confirmation of Consent form

(e) Confirmation of Orientation Meeting

(f) Confirmation of Individual Employment Plans (initial)

(g) Referral details

(2) Colleges will also maintain, and make available upon request, Individual client files that include at a minimum:

(a) Signed Consent Form

(b) Individual Employment Plan (Initial)
(c) Referral documentation

When we begin monitoring we will ask to see student files and records to make sure you have all this information/forms.
