Very cool, fun idea. It works really nicely.
There's a really cool tool called Twine that is specifically made for making interactive, choose your own adventure-type games. It's pretty fun to use and can make some really neat looking games. Fantastic idea!! You could do a presentation at a library conference, it’d be super popular.

Another library did a Harry Potter theme “escape room” similar to this, also in Google Forms.

This clever, short info literacy tutorial crossed my radar this morning, and I thought it was pretty fun, and worth sharing.

Kind of like an old click-and-pick your adventure game.

Would be a nice idea to flesh out in longer form, maybe with old-style pixel graphics.
