Mindy:  Please consider using the Standard for Libraries in Higher Education document created by ACRL.  Its a wonderful framework, and also helps align with accreditation.  The focus on multiple methods for assessment allows for a great deal of flexibility for the library.  And the bonus is you can also do a connection to campus outcomes.  I'm one of ACRL's trainers on the Standards and happy to help answer any questions.  Here is the link to the doc:

At Skagit, we have not done a program review of the library since 2010.  We are planning to do one next year.  My VP asked me to inquire if anyone has a good process for reviewing the library.  I have created a list of elements to include in the review, but would be interested in learning about process.
