I've developed the student intake form using Formstack and provided the link for students to access it; here is the link as an example: LACES Student Intake Form<https://lwtech.formstack.com/forms/laces_student_intake_form>. I can download all responses as an Excel file, which allows me to manipulate the data to match the partial intake requirements. I'm still working on the upload process but have successfully used this method to upload intake forms into WABERS+ before, and I'm hopeful it will work for LACES as well.

Have you looked into using Formstack for similar purposes?

Hi all,



I am curious as to what everyone's system may be for collecting the race and ethnicity information from students.



We are close to developing an online student submissions portal for students to submit the LACES partial intake form digitally. However, my understanding is that student answers for the race/ethnicity data cannot be directly uploaded into LACES quite yet due to system errors, and we must delete any columns to do with race/ethnicity from the downloaded .csv file prior to processing and instead enter this data in manually for each student.



Has anyone come up with a different method for collecting this data? We're exploring options over here and I'd love to learn what others might be doing.
