[Cato] campus contact survey

Hello,


I know a few IT Access Coordinators that aren't working with a team or effectively distributing information. So, I'm going to attempt to track down the folks that I think should be on their team in order to better share resources and training opportunities that are relevant to specific areas.


Here's the survey I want to send - can you think of anything to add or reword? I'm impatient, so will probably send this on Monday or Tues. Please take a look before then.
