[BFET] Required Forms Due Reminder

Good Morning BFET Team!

Sending you a reminder that Monday, September 10th, 2018 is the deadline to submit your alternate forms if you are planning on using them for FFY19.

Forms that must be submitted for approval are:


·         Participant Reimbursement Form

·         Release of Information/Consent Form

·         IEP

·         BFET Referral Form

Please note that you must receive written approval before using any alternate forms. Should you have your forms in by the deadline you will receive approval status by October 1st.
