God morning.
I have attached a letter from DSHS regarding the opportunity to request 100% funds mid-year, along with the FFY18 Exhibit C 100% Carryover Funds Request Form and the Exhibit D 10% Carryover Funds Performance Form.

The letter states that a response is needed no later than Monday, November 29th. This is a very tight timeline, especially with this being the week of Thanksgiving. Hopefully, you've had opportunity to think about this already. Please submit your request and performance forms to me no later than noon on Monday, November 29th.

100% funds and the performance increase from receiving those funds should be for the time period of March-September 2018. For us, that would mean planning for Spring quarter, Summer quarter, and September 2018.

A few key things to keep in mind as you develop your requests:

1.       Don't forget to input your college name on both forms.

2.       Additional funds require additional performance.

3.       The average cost per person should be approximately the same as your original cost per person.

4.       The % of Success cell (K6) on the Performance form should be no less than 60%.

5.       Carryover 100% funds can be used for administrative costs and tuition only, no participant reimbursement expenses.

6.       Tuition on the Exhibit C Funds Request form should be input under Client Services.
