I have a really short form I use that has the expectations of the RCW in them, which is to ask about satisfaction and about how can trainings be used in work or education at the school.  The other three questions on my sheet are about the three learning outcomes from whatever the program or training I am doing are. We provide it in hard copy and we provide it digitally through a link and a QR code!
