[BFET] DSHS Case Note Audit Report for June 2017

Good afternoon.
I have attached the June Case Note Audit Report.

As you review the report, you may notice an increase in your number of missed notes. There are two reasons for this:

1.       DSHS has received training on the evaluation standard of case notes, so all staff are consistent in their evaluation methods.

2.       The report is now separating findings into cases with no notes and cases with insufficient notes, though both categories are lumped into "missed case notes" on the report.

The evaluation standard that is being used looks for the following information to determine a sufficient note:

1.       Does it state how the student is progressing?

2.       Does it state how this was determined, e.g., by email, through in-person contact, a phone call, a hard-copy or online progress report form completed by the student, etc.
